Format of Consumer Complaint

Dear Applicant,

I am attaching a PDF to this article containing the format of the consumer complaint. I have also explained below how to properly understand and use this format while preparing your complaint. You should place the complaint format after the index page of the complaint and in the attached pdf, I have also included the format of the index and the format of the consumer complaint.

If you are the complainant and you do not want to engage and advocate, you may file the consumer complaint on your own before the Consumer Commission. You should draft the complaint strictly according to the provided format and then file it before the Commission and please note that in the consumer cases, both online and offline filing are mandatory. If you do not file the complaint offline, your case will not be listed before the Consumer Commission.

First you must draft the consumer complaint along with the attached relevant annexures/documents as per the format provided in the PDF. After preparing the contents, you should create the separate PDF file for each section, such as

  • Index
  • Format
  • List of documents
  • consumer complaint along with affidavit.
  • Documents/annexures such as bills, receipts and bank transaction details, etc.

Important Point:- If the case of the consumer is complicated, then the consumer should hire/appoint an advocate, as they can properly file the complaint and help the consumer at every stage of the case.

Here, I will explain to you how to file all the sections of the consumer case format.

In S No. 1, the name and address of the consumer/petitioner/appellant with mobile number and email address.

  • Name: Full name of the consumer/petitioner/appellant
  • Address: Full residential address with City, State, and Pin code
  • Tel. (Landline No.) if available; otherwise, write N/A
  • Mobile Number. Your Mobile Number
  • Email ID: your Email ID

In S No. 2, Total Consideration Paid, the value of the goods/service paid by the complainant as consideration exceeds _______ rupees. 

  • Total consideration paid: Rs.XXXX (Rupees XXXX Lakh only)
  • Page No.   55X
  • PAge No.   12X
  • (Mention the page number where the value is written in prayer). 

In S No. 3, Nature of the Dispute, Select as per the category of your case. 

  • Insurance
  • Housing
  • Medical Negligence
  • Banking/Financial Services
  • Electricity/water/telecom services
  • Education
  • Transport
  • E-Commerce
  • Other (specify) _____

In S. No. 4, The project name related to the housing category indicates the page number where mentioned. 

  • Project Name:  ABC Residency House Project
  • Page Number mentioned: – Page No.  06

If you do not know the project name, then write N/A.

In S. No. 5, please indicate the relief claimed in the matter such as 

  • Refund of the principal amount
  • return fo the amount which paid
  • interest on the principal amount
  • possession of the property
  • compensation for the delay or deficiency in service.

If you are not know, Mention N/A

In S No. 6, please specify the Application section 

  • Section 35(1)(a) = Complaint filed by the consumer
  • Section 35(1)(b) = complaint filed by any recognised consumer association, whether or not the consumer is a member.
  • Section 35(1)(c) = complaint filed by one or more than one on behalf of the numerous consumers having the same interest (class section)

In S No. 7, detailed of the mending consumer complaint if any

  • If the consumer dispute is related to the housing or land development project, please provide the details of any pending consumer complaint, if any, in respect of the same project.

In S No. 8, details of the pending/disposed complaint involving the identical question of law. 

  • Mention the details of any pending or disposed off consumer complaints, if any, which involve an identical question of law.

If you are not know, Mention N/A

In S No. 9, date of action

  • Date of cause of action: Ex. 15/08/2025
  • Mention the exact date when the issue first arose.
  • If the problem is ongoing, then write continuing to date.

In S No. 10, if there is any delay in computing the period of limitation.

  •  Please specify the delay date if there is any delay in filing the complaint. Delay days are computed from the date of the cause of action.

If there is no delay, then mention Nill 

In S No. 11, application of condonation of delay

  • Please indicate that the English translations of all documents have been filed along with the complaint.

  • Full complete legal name of the opposite party
  • all available contact details, as notice may be sent by email or phone

In the last verification, if counsel or an authorised representative is filing, they should sign and mention their designation and always mention the place and date next to the signature.
